Actors

Clinician

A practicing clinician learner who is engaged in activities to measure competence on an ongoing basis (such as Maintenance of Certification or Competence)

Portfolio

A system that records the clinician's evidence of competence using a competency framework.

Self assessment system

A system running activities designed to help the clinician evaluate their own competence. These activities may be mapped to competencies within a competency framework.

Quality improvement system

A system running activities designed to help the clinician measure and improve performance. These activities may be mapped to competencies within a competency framework.

Learning Management System

A system offering learning activities to clinicians. These activities may be mapped to competencies within a competency framework.

Outcomes database

A database that allows clinicians to record their clinical outcomes and compare their outcomes to those of their peers. There are several such databases in sugery.

Assumptions

An organization, such as a certifying board, is tracking the clincian's activities against a competency framework.

Description

The clinician engages in a self-assessment, quality improvement, or learning activity that meets a requirement for a competence assessment program and addresses a specific competency within the competency framework used by that assessment program.
The self-assessment, quality improvement, or learning management system sends an electronic message to the clinician's portfolio indicating the clinician's completion of the activity and the result or score relative to each relevant competency.

OPTIONAL
The self-assessment, quality improvement, or learning activity may direct the learner to other activities based on gaps in competence.
After such remediation, a clinician may return to a QI activity to measure performance again and document any improvement that may occur.
The QI system sends a message to the clinican's portfolio, which documents the change (or no change) in performance.

Transactions

The self-assessment, quality improvement, or learning management system sends an electronic message to the clinician's portfolio indicating the clinician's completion of the activity and the result or score relative to each relevant competency.
A system directs the learner to an activity based on a gap in competence.

Exceptions

The portfolio system can't accept the message (wrong competencies used, wrong format)
