We wanted to propose two changes to Member Professional Schema for our next call.

 

  1.  Adding a new enumeration value “Designation” to CertificateIssuanceOccurrence element.  This new value “Designation” will be used for certification records with CertificateIssuanceFocus which defines a focused area of an certificate.  Focused practice in Hospital Medicine is a designation certificate and not an Initial or a Recertification.  Designation value is requested to give acknowledgement to the physicians who are focusing their practice on the care of hospitalized physicians and have completed certain years of practice in addition to an exam, and therefore the value of initial or recertification cannot be used.

2.  Adding new enumeration value “Converted” to CertificateIssuanceStatus element.  This new status will be used for certificates which are being converted to a new certificate.  For example, a lifetime physician who is participating in MOC, their existing active certificate can be converted and issued a different certificate with a new name.  “Converted” status notifies credentialing community that the old certificate is converted to a new one.

Additionally, the AAMC would like to propose a change to the EducationStatus subelement of EducationInfo. 

 

In working with NBME and our members, we have found that the static list of statuses is too restrictive.  I propose that we change the data type from Restricted to Non-null string, and change the description to suggest possible values.

EducationStatus

This is the sub-element of the EducationInfo element. It defines that staus of the individual in relation to the educational program. Valid values include: Active, Graduated, Leave of Absence, Withdrawn, Dismissed, and Deceased.

EducationStatus

This is the sub-element of the EducationInfo element. It defines that status of the individual in relation to the educational program.  Values are expected to be succinct descriptive words or phrases such as  Active, Graduated, Leave of Absence, Withdrawn, Dismissed, No Longer Enrolled.

The reason for making this less restrictive is because for different use cases we need to provide more or less specific status values.  For example, internally we distinguish between Withdrawn and Dismissed (which connote voluntary or involuntary departure) but in many cases we translate that to something like “No Longer Enrolled” to obscure that distinction.  It seems like every time we discuss this element with respect to a data exchange, we come up with a new status or a new business rule, so I’m not optimistic about being able to come up with a future-proof list of restricted values.
