  • All other teaching/instructional/educational activity such as continuing education, outreach, and development of teaching manuals or special instructional formats.
  • All “formal” advisees, degree sought, role as advisor, and completion/status of degree.
  • List of all individuals trained/mentored/advised (post-doctoral fellows, trainees, etc). Include current position of these individuals.

Number the publications under each heading.  Each publication should include the full title, complete list of authors (underline/bold your name), journal title, journal volume, page numbers (inclusive), and date of publication. In addition, the candidate must insert at the bottom of each item a notation that includes:  impact factor of the journal, citation index for the article, and for each multi-authored article or collaborative project, the contribution of the candidate to the item.
