Do not include information such as date of birth, gender, marital status, spouse, children’s names or ages, etc.
Use reverse chronological order – present to past – for the following sections:
List year, school, and degree for each.
List any type of special leaves, sabbatical leaves from academic appointments for research. Include the dates, type of leave, and location.
CERTIFICATION AND LICENSURE
Include year and type of Board Certification, Fellowship certification, BLS/ACLS, etc.
Licensure: List date and states for medical licenses, DEA registration, etc.
List dates and types of military service, including Reserves.
List dates, titles and institution of all academic appointments

List dates and names of journals or institutions.
Ad hoc reviewer of manuscripts or grants
Membership in programs such as the Cancer Center or Cellular and Molecular Biology
List current, pending and past and grant support (in separate subsections), with dates and funding amount (specify direct cost or total cost, and annual or total project period)
For each grant include:
Name of Principal Investigator
Candidate’s role on the grant (such as overall PI, site PI, or candidate’s specific role)

TEACHING AND MENTORING ACTIVITIES
- All courses taught, including course number, title, brief description, quarter/semester, role, and number of students enrolled.
- All other teaching/instructional/educational activity such as continuing education, outreach, and development of teaching manuals or special instructional formats.
- All “formal” advisees, degree sought, role as advisor, and completion/status of degree.
- List of all individuals trained/mentored/advised (post-doctoral fellows, trainees, etc). Include current position of these individuals.
- Honors/awards received for teaching
- Anything else that relates to teaching
EXTRAMURAL INVITED PRESENTATIONS
List invited international, national, and local presentations, lectures, and seminars. Include the titles, institution, and date. Number the presentations under each heading.

List completed Publications of Scientific Journals under the headings below.
Number the publications under each heading. Each publication should include the full title, complete list of authors (underline/bold your name), journal title, journal volume, page numbers (inclusive), and date of publication. In addition, the candidate must insert at the bottom of each item a notation that includes: impact factor of the journal, citation index for the article, and for each multi-authored article or collaborative project, the contribution of the candidate to the item.
